Hospital Clínica Bíblica and Hospital CIMA San José are the two accredited private hospitals with international patient departments, both with 24-hour emergency rooms. Hospital La Católica in Guadalupe and Hospital Metropolitano cover the lower-cost private tier. Public emergencies go to the Caja network, reached by calling 911.

Independent laboratory chains run 20 to 55 branches across the Central Valley, publish their prices online and offer home collection. They undercut hospital labs substantially and need no referral.

Private care needs no referral. You phone or message the specialist directly, and same-day or next-day slots are normal. Private emergency rooms are walk-in and open around the clock.

The Caja starts at your assigned local clinic. Enrolment ties you to an EBAIS team by home address, and that doctor must refer you onward. Skipping the step is not possible, so private care is the fast route in your first months.

Ask for a presupuesto before non-urgent work. Private hospitals run a formal quotation service, and an emergency bill is itemised rather than a flat fee. Get the estimate in writing, because the total is driven by the workup.

Pharmacies

Antibiotics have never been sold over the counter here, and since 2025 they run through the national digital prescription system. Psychotropics and narcotics sit in the same system. The prescriber sends you a code by email or message, which you present at the pharmacy. A foreign prescription cannot generate that code, so anything controlled needs a locally registered doctor. Pharmacists otherwise have wide latitude to advise, and almost all of them administer injections under a professional protocol. No chain pharmacy opens all night, so the overnight route is a private hospital pharmacy.
